I was asking my developer for the same but they created a new html form instead of google form. Nice looking for more tips from you.

Thanks Moeez. I dint know about Google form plug in and so created stand alone HTML page with the help of Google form . Stand alone HTML page creation also helped me to customize thank you page and have the benefit of Google sheet fill up with leads collected from the form.
